Reject real patients with the shutter — safe. Reject confirmed fakes — required. Appearance anomalies (window)
These fail the live monitor — no tools needed, though twitching may need four to five seconds of watching.
Three Eyes
Three glowing red or orange eyes — sometimes real eyes still visible beneath. Easiest reject in the game; shutter immediately.
Hollow Face
Dark empty sockets, hunched posture, optional twitching. Not the same as a sad injured patient — sockets are fully vacant. Often linked to death rituals if admitted.
Twitching
Arms, neck, or head jerk while standing still. Some variants look normal at the window but twitch only on CCTV — always finish camera sweep.
Realistic / Crooked Face
Hand-drawn human smiles, mismatched popped eyes, crooked grins with sharp teeth. Voice may sound distorted.
X-rayed Face (Human Teeth)
Realistic human teeth on an animal muzzle — instant reject.
Staring
Patient tracks your movement or locks onto cameras with unnatural head rotation — treat as hostile intent.
Photo anomalies (polaroid)
Wait for the print to fully develop before comparing to the window patient. Picking up some cursed prints drains ~10 sanity — inspect on the desk first.
Incorrect Photo
General mismatch — the animal in the print is not the animal at the glass.
Different Eyes / Different Ears
Subtle swaps: wrong pupil shape, bear ears on a rabbit, horn count changes. Check ears even when the face looks fine.
Static Photo
Grainy TV-static overlay on the polaroid. Window face can look perfect.
Cursed Photo
Melted grins, bloodshot horror faces, distorted features only on paper.
If window, photo, and CCTV disagree, reject. Community shorthand: any layer wrong means anomaly.

Black Eyes

Unnatural Body
Stretched, boneless, or warped limbs — limbs longer than the hallway should allow.
Void Body
Pure black silhouette on feed — no color detail. Instant shutter.
Staring at Cameras
Patient faces the lens instead of the desk while walking or waiting.
Camera Twitching
Still at window, twitching only on feed — leave CCTV open several seconds.
Disguised Shapeshifter
Normal at window; true monster form visible on CCTV once they reach check-in. Check both lobby side angle and check-in cam.
Mismatching Ears / Mismatching Eyes
Small ear or eye differences between window and camera — easy to miss when rushing.

Shapeshifter (after admit)

Slimebert
Slimebert (community name) is a roaming slime entity during secret slime shifts, not a patient at the check-in window.

- Assign one player to monster cleanup while others stabilize real patients
